Athletic Pre-Participation Form
The Athletic Pre-Participation Form must be filled out by their doctor and turned in to the coach before the child tries out for any sport.
Athletic Consent and Liability Form
The Athletic Consent and Liability Form must be filled out by a parent/legal guardian and turned in to the coach before the child tries out for any sport.
